Our “Post-Acute to Home” (PATH®) short-term rehabilitation program, in partnership with Aegis Therapies®, is a bridge to aid in recovery from illness, injury or surgery after a qualifying hospital stay. The program provides short-term rehabilitation with speech, occupational and physical therapies, as well as skilled nursing services to help seniors in Kirkwood, MO, and greater St. Louis recover faster so they can get back to their daily routine.

Our team dovetails PATH® rehabilitation therapies with skilled nursing care to meet each patient’s personal and medical needs. Regular family meetings keep all stakeholders informed, and help speed recovery. PATH® steps to

ADMISSION
WHAT YOU NEED TO KNOW

Start by contacting your hospital social worker or discharge planner. Tell them you’ve selected The Glen at Aberdeen Heights for your rehabilitation stay. The social worker will contact us on your behalf to check availability and start the admissions process.

Our PATH® team reviews medical and treatment information to assess whether those referred to our program are appropriate for admission. When possible, a nurse visits the patient while they are still in the hospital. During the review, the information listed here will be gathered — from there, the social worker will guide you through hospital discharge and arrangements to be admitted to PATH® rehabilitation.

PREPARING FOR YOUR STAY

We recommend gathering several key items before your scheduled procedure or having a loved one gather them during your hospital stay. Download our packing checklist of recommended items to include.

Most PATH® patients follow the Medicare guidelines for skilled care, which required a three-day qualifying stay in an acute care hospital. For more information, check out this helpful guide.
